INSTRUCTIONS

Complete all required sections before submitting your parts request or return. Include vehicle, fuel system, and part details so the Parts Department can process your request accurately. For returns, include this completed form with the returned parts.

PARTS RETURN POLICY

Parts must be returned within 30 days andin resalable condition. This form must be filled out and returned with all parts in original packaging to be eligible for credit. A 15% restocking fee may apply. No returns on installed or used parts. Special order parts may not be returned. Freight is not credited unless pre-approved by Parts Department management.
